Making the Healthy Choice the Easy Choice
Creating Healthy Communities (CHC) is committed to preventing and reducing chronic disease statewide. Through cross-sector collaboration, we are activating communities to improve access to and affordability of healthy food, increase opportunities for physical activity, and assure tobacco-free living where Ohioans live, work and play. By implementing sustainable evidence-based strategies CHC is creating a culture of health.
Creating Healthy Communities is a five-year grant project from the Ohio Department of Health. The first year of the grant will focus on capacity building which includes expanding the membership of the Get Healthy Knox County coalition which will now become the CHC Coalition. The first year will also include conducting a CHANGE tool assessment with community agencies, schools, businesses and organizations to identify Knox County’s resources and areas of improvement.
