We are so excited to announce the Knox County Community Health Center has joined forces with Moore Family Practice and the Walk-In Clinic! Official management of both practices under Knox Public Health started on Monday, January 3, 2022.
Less than a year before the merger, business owner Simona Moore, CNP approached Health Commissioner Julie Miller about the possibility of her medical practice joining the Health Center family. Knowing the needs and priorities of her patients, Simona recognized the benefits of an federally qualified health center (FQHC) - such as more services, in-house referrals, and financial assistance.
Knowing full well the top-notch reputation Simona and her team has in the Knox County community, Knox Public Health and the Knox County Community Health Center felt the merger was meant to be. By the very first week of 2022, the merger was successfully completed.
The Benefits & Differences
At first glance, there will not be any difference. Patients will see the same providers and receive the same great quality care they are accustomed to. Simply put, this new partnership doesn't change the appointment or the compassionate care your providers will give
However, the merger will allow Moore Family Practice and the Walk-In Clinic to expand treatment options, provide additional services, and resource referrals not previously offered. Now as part of the FQHC family, patients will have new payment opportunities to ensure that cost of service is never a barrier to high quality and consistent healthcare. Payment plans, sliding fee scale, and nominal fees are now part of the financial landscape at the two offices.
Additionally, transportation to and from our facilities is now FREE. Moore Family Practice and the Walk-In Clinic have been added to the locations where Health Center patients can receive free transportation for healthcare services. Add that perk along with the plethora of services available at the other three Health Center locations in Knox County (Mount Vernon, Danville Community Health Center, and Knox County Dental), and we can now meet your dental and counseling needs along with the medical services we provide.
As the Health Center expands, its first and foremost goal is to serve your family and care for their healthcare needs. By adding the excellent practitioners and service provided by Moore Family Practice and the Walk-In Clinic, that reach has grown tremendously.

Simona Moore, CNP
Simona graduated from The Ohio State University with her Bachelors in Science of Nursing in 1990. Upon graduation, she spent five years working as a registered nurse at The Ohio State Medical Center. From 1997-2000, Simona earned her Masters of Science in Nursing, and Adult Nurse Practitioner from The Ohio State University. She also earned her Family Nurse Practitioner certification from Otterbein College, now Otterbein University. Simona has practiced as a Nurse Practitioner in the local community for nearly 20 years. Simona is a board member with the Winter Sanctuary (alongside Health Commissioner Julie Miller), which serves the homeless population of Knox County. In her spare time, she enjoys spending time with her grandchildren, tending to her garden, and quilting.
Jordan Moore, CNP
Jordan has been with Moore Family Practice since the beginning in 2013. He is our newest nurse practitioner, graduating from Chamberlain University in the spring of 2019. Prior to joining the practice, Jordan spent the past 15 years in the military. After several deployments, he transitioned into the healthcare field. Jordan graduated Cum Laude from Otterbein University in 2016 with his Bachelors in Science of Nursing. Jordan continues to serve the United States Army Reserve as an Army Nurse. In his free time, he enjoys spending time with his family, working on his farm, and staying physically active.
Sarah Rice, CNP
Sarah graduated from Chamberlain University with a Masters of Science in Nursing and earned her certification as a Certified Nurse Practitioner in 2016 through the American Academy of Nurse Practitioners. She is also board certified with the Ohio Board of Nursing as a Registered Nurse. Her practicing concentration is in family practice. Sarah has nine years of Emergency Room and Intensive Care Unit (ICU) experience. In her spare time, she enjoys riding her horses and spending time with her family.
Jill Hunter, CNP
Jill has been in the medical field for almost 30 years and has worked at Moore Family Practice since 2017. Most of her time has been spent in the critical care field, which has assisted her in caring for her patients in primary care. She received her Master’s Degree and Family Nurse Practitioner certification from Chamberlain College of Nursing in 2016. Jill enjoys building a relationship with her patients and helping them with their health and wellbeing. She has been married for 28 years and has 3 daughters. In her free time she enjoys spending time with her family, crafting, sewing, and reading.
