The Knox Health Planning Partnership (KHPP) is responsible for the development of the Community Health Assessment (CHA) and the creation, implementation, and monitoring of the Community Health Improvement Plan (CHIP). These documents are meant to serve the Knox County community as resources for improving health and wellness in our community. KHPP is comprised of over 15 community agencies representing all sectors. The mission of KHPP is to improve health and quality of life by mobilizing partnerships and taking strategic action in Knox County
What is a CHA?
A Community Health Assessment (CHA) is a community-wide, survey based, report that outlines the major health issues in Knox County by collecting primary data from county residents. The CHA process starts by designing a survey that asks residents a broad range of health-related questions including: physical health, mental health, health behaviors, quality of life, and demographics.
When all the surveys are collected, they are analyzed and a report is created. That report contains results of all survey questions, demographic information, state data, and various secondary data sources related to the health of Knox County. KHPP uses this report as the foundation for the Community Health Improvement Plan.
What is a CHIP?
A Community Health Improvement Plan serves as a blueprint or planning guide for community agencies to help address health concerns identified in the CHA. Partnership members review the CHA data and rank the top ten key issues to help narrow the focus of the CHIP. Three priority areas are selected from the top ten key issues for the CHIP. Those three priority areas are the basis for the CHIP.
What is this important?
CHAs and CHIPs are important for two reasons. The first is, they are state mandated. Every county is Ohio is required to complete a CHA and CHIP every 3 years. Secondly, and most importantly, the CHA and CHIP are both a vital part to the work done by not just the health department but with community partners throughout Knox County. The information learned from the CHA provides agencies with a baseline of what health and wellness look like in Knox County. The CHIP gives everyone a common planning document and mission to work toward to reinforce that this is truly a community effort.
